Short answer: No public national source reports certified hand therapist (CHT) pay on its own. The best baseline is federal data for the two licenses every CHT holds. The U.S. Bureau of Labor Statistics puts the May 2025 median at $100,330 for occupational therapists (opens in a new tab) and $102,760 for physical therapists (opens in a new tab). A CHT offer starts from that baseline and moves with your setting, your market, and how hard the credential is to find near you.

I am a recruiter, not a therapist. I will not tell you how to run a hand clinic. What I can tell you is what the public pay data actually says, why the numbers you find online disagree, and what we see move a CHT from "not interested" to "let's talk."

What does a certified hand therapist make?

Start with what is verifiable. The CHT credential is held by occupational therapists and physical therapists who meet extra requirements. The Hand Therapy Certification Commission (HTCC) (opens in a new tab) requires at least three years of practice as an OT or PT and 4,000 hours of direct practice in hand therapy before a candidate can sit for the exam. So the right pay floor is the OT and PT market, not a general rehab aide or assistant market.

Here is the federal baseline, from the BLS Occupational Outlook Handbook (May 2025 data):

BLS does not break out CHTs as their own occupation. Anyone who quotes you a precise national CHT salary is quoting an estimate, not an official count. That is also why we say on our certified hand therapist recruiting page that we do not publish a CHT specific salary figure.

Setting matters even inside the same license. BLS reports that OTs in home health earned a median of $106,300 (opens in a new tab), while OTs in therapy offices earned $97,940 (opens in a new tab). For PTs, hospitals came in at $106,800 (opens in a new tab) and therapy offices at $97,160 (opens in a new tab). Most hand therapy happens in outpatient clinics, orthopedic practices, and hospital hand programs, so know which bucket your role sits in before you benchmark.

Why do hand therapist salary numbers online disagree?

If you search "hand therapist salary," you get a page of salary sites, and they do not agree. That is not a typo on anyone's part. They measure different things.

  • Job board estimates are modeled from job postings and user data. ASRA, for example, showed an average of $84,619 a year for a CHT (opens in a new tab) when we checked on October 27, 2026. That sits below the BLS median for OTs and PTs overall, which tells you how much these models depend on which postings they happen to scrape.
  • Self reported figures come from people who chose to enter their pay. Small samples swing hard, and some pages stay up for years without fresh data.
  • Federal data from BLS is a large employer survey, but it stops at the license level.

None of these is the number a CHT you want will compare your offer against. They compare it against their current job and whatever a competing clinic just offered them. Treat salary sites as a sanity check, not a pricing tool.

What moves a CHT offer up or down?

These are the levers we hear about in real conversations with hand therapists and the employers who hire them. None of them require you to guess at a national CHT average.

Setting and caseload

Hospital hand programs, physician owned orthopedic practices, and private outpatient clinics pay differently, as the BLS setting data above shows for OTs and PTs. CHTs also ask how many patients they will see per day and whether time for custom orthosis fabrication is actually scheduled. A caseload that looks fine on paper can feel like a pay cut if fabrication time is unpaid.

The credential itself

The CHT is not a weekend course. HTCC reports an ultimate pass rate of about 65% (opens in a new tab) for candidates across multiple attempts, and certified therapists must recertify every five years (opens in a new tab). Many employers add a differential or bonus for the credential. If you do, say so in writing. If you do not, expect to explain why.

Local scarcity

The pool is small. HTCC reported 7,665 active CHTs in the United States (opens in a new tab) in the 2026 ASHT annual report. Spread that across every state and plenty of markets have only a handful of CHTs within commuting distance. Scarcity in your area does more to your offer than any national average.

Schedule and growth

Four day weeks, protected admin time, a lead or program building role, and support for continuing education all show up in how a CHT weighs an offer. For some, these matter as much as base pay.

Why posting a salary range does not reach passive CHTs

Posting a competitive range is the right thing to do. It just does not solve the reach problem. Most CHTs who would be a great fit for your clinic are employed, busy, and not scrolling job boards. They will never see the post.

HTCC itself notes that while many employers prefer to hire a CHT, some will hire an inexperienced person and train them (opens in a new tab). That is a real option, and our employer guide to hiring a certified hand therapist covers how to describe the role either way. But if you need someone credentialed now, you have to go find them, one conversation at a time.

How to build an offer a passive CHT will take

  1. Benchmark from the license, then adjust. Start with BLS OT or PT pay for your setting and metro, then layer in the credential and local scarcity.
  2. Write down the credential differential. Vague promises of "competitive pay for certification" do not move people who already have a good job.
  3. Show the caseload honestly. Patients per day, visit length, and scheduled fabrication time.
  4. Name the growth path. Program lead, surgeon partnerships, mentoring newer therapists toward the CHT.
  5. Move fast. A CHT who agrees to talk is usually talking to more than one employer. A slow interview loop is the most common way we see good offers lose.

We are recruiters who specialize in hard to fill clinical roles, including occupational therapist recruiting and physical therapist recruiting. For CHT searches, we reach hand therapists directly and confidentially, including the ones who are not looking. Our hand therapist jobs page is where those therapists book a private career call with us.

The employer pays our fee. Candidates are never charged. Fee terms and the replacement guarantee are set out in your engagement agreement before any outreach starts.

FAQ

How much does a certified hand therapist make?

No federal source reports CHT pay on its own. The closest official baseline is BLS data for the two licenses CHTs hold: a May 2025 median of $100,330 for occupational therapists and $102,760 for physical therapists. Actual CHT offers vary by setting, market, and how scarce the credential is locally.

Do CHTs earn more than general OTs and PTs?

Public national data cannot answer that, because BLS does not separate CHTs. Many employers do add a differential or bonus for the credential. Ask any CHT you interview what their current package includes before you set your number.

How many certified hand therapists are there?

The Hand Therapy Certification Commission reported 7,665 active CHTs in the United States in the 2026 ASHT annual report, and more than 8,000 worldwide.

How do you recruit a certified hand therapist?

Most strong candidates are already employed, so job posts alone rarely reach them. Direct, confidential outreach with a clear offer, an honest caseload description, and a fast interview process works best. A specialist recruiter can run that outreach for you.
