About the Opportunity
Engaged Headhunters has been retained by a Confidential Five-Star Post-Acute & Skilled Nursing Operator in Coastal Virginia (Virginia Beach / Hampton Roads) to recruit an experienced Licensed Nursing Home Administrator (LNHA).
Rather than walking into an under-resourced turnaround with an empty leadership bench, you will step into a well-capitalized 120+ bed post-acute rehabilitation and skilled nursing center with a tenured Director of Nursing (DON), full-time MDS Coordinator, Director of Rehab, and Business Office Manager already in seat. Regional operations, clinical reimbursement, and HR support are structured to let the Administrator focus on hospital referral growth, survey excellence, and team culture.
Compensation & Executive Package
- Base Compensation: $145,000 to $185,000 annualized base salary (commensurate with multi-facility or high-Medicare-census LNHA tenure)
- Executive Bonus: Up to 25% annual performance bonus paid quarterly across transparent census/payor-mix, CMS Five-Star quality, state survey, and EBITDA milestones
- Sign-On & Relocation: $15,000 sign-on or coastal Virginia relocation package available

Core Executive Responsibilities
- Facility P&L & Operational Leadership: Hold full operational and financial accountability for a 120+ bed SNF, managing operating budgets, PDPM Medicare Part A / Managed Care case-mix optimization, and labor efficiency without nursing registry dependency.
- Regulatory & CMS Five-Star Stewardship: Partner closely with the Director of Nursing (DON) to maintain continuous VDH / CMS survey readiness, lead QAPI governance, and protect Five-Star Quality and Staffing ratings.
- Acute-Care Hospital Referral Growth: Cultivate direct relationships with local hospital discharge planners, orthopedic/cardiac surgical service lines, and ACO networks across Hampton Roads to drive high-acuity short-term rehab admissions.
- Department Head Retention & Culture: Lead daily standups and mentor department directors across nursing, therapy, dietary, environmental services, social work, and admissions.
Required Qualifications
- Licensure: Active, unencumbered Licensed Nursing Home Administrator (LNHA) license in the Commonwealth of Virginia (or active NAB/HSE credentials eligible for immediate Virginia endorsement).
- Leadership Track Record: 3+ years of successful tenure as Administrator of Record (LNHA) in a licensed skilled nursing facility (SNF) or Continuing Care Retirement Community (CCRC) with verifiable state survey and financial performance.
- Reimbursement Fluency: Strong operational command of Medicare PDPM, Virginia Medicaid, managed care contracting, and SNF staffing matrix management.
